Summary
This study looked back at past patient records to see if a computer program could learn to predict which patients are most likely to be readmitted to the hospital within 30 days after being sent home. Researchers used data from 372 patients who had been treated at home through a special hospital program. The goal was to create a tool that could help doctors identify high-risk patients earlier during their care.
