Can extra Check-Ins keep liver patients out of the hospital?
NCT ID NCT05733832
First seen Jun 25, 2026 · Last updated Jun 27, 2026 · Updated 2 times
Summary
This study tests a special follow-up program called the Transitional Liver Clinic (TLC) for people with advanced liver disease after they leave the hospital. The program includes extra phone calls or video visits and more attention from liver specialists. The goal is to see if this extra care reduces hospital readmissions within 30 days, improves quality of life, and boosts patient satisfaction. The trial plans to enroll 1,000 adults with cirrhosis or alcoholic hepatitis.
What this could mean
Our plain-language read of the trial. This is informational only — not medical advice or a prediction.
Active substance
Transitional Liver Clinic (TLC) - a care model with extra follow-up calls and visits
What this could lead to
If successful, this could show that a simple, organized follow-up program helps people with advanced liver disease avoid returning to the hospital and feel better after discharge.
What could go wrong
This is a care model, not a new drug or treatment, so it won't cure liver disease. The results may vary across different hospitals or patient groups, and the study is still recruiting.
