This study uses the Swedish Heart Failure Registry to identify people with heart failure and reduced ejection fraction who may need medication adjustments, iron deficiency screening, device therapy, or physiotherapy. Researchers will compare outcomes for 500 patients who receive this registry-based screening and referral to 500 patients receiving usual care. The goal is to see if this approach helps more patients get the treatments they need.
