Summary

This study aims to improve treatment for people with heart failure by using a Swedish registry to find those who need medication changes, iron deficiency screening, or heart devices. About 1,000 participants will be split into two groups: one gets extra support to optimize their care, while the other receives usual treatment. The goal is to see if registry-based screening helps more patients receive the right therapies.
