Summary

This study tests whether having extra appointments with a heart specialist at a local GP practice helps people get better treatment for heart, kidney, metabolic, and lung conditions. 138 high-risk adults will either receive standard GP care or additional specialist reviews to optimize their medications according to national guidelines. After 6 months, researchers will check if the specialist group had more medication adjustments and better control of risk factors like blood pressure and cholesterol.
