Summary

This study tests a program called NAVIGATE-Kidney for people with advanced chronic kidney disease. The goal is to help them start kidney replacement therapy (dialysis or transplant) without needing a temporary catheter, which can lead to infections. About 448 adults will take part to see if the program improves health outcomes and reduces disparities.
