Summary
This study tests whether a one-year program led by heart failure nurses, using online lessons and a mobile app, can help people with heart failure avoid repeat hospital stays and feel better. About 200 adults who were recently hospitalized for worsening heart failure will either get the program or standard care. The program includes training, monitoring, and connecting patients to a heart care network.
