Summary

This study tests a program called I-TRANSFER-HF for people with heart failure who need home care after leaving the hospital. The program combines early nurse visits at home with quick follow-up doctor appointments to prevent patients from returning to the hospital within 30 days. Researchers will compare outcomes before and after the program is adopted at hospitals and home health agencies across the country, and interview staff to see how well it works in real life.
