Summary
This study tests whether a coordinated care program called I-CONNECT can improve the health and well-being of older adults (65+) who have depression or bipolar disorder and at least two chronic physical conditions like diabetes or high blood pressure. Participants receive personalized care coordination, medication review, and regular follow-up from a team including their GP and psychiatrist. Researchers will compare their outcomes to those receiving usual care, focusing on quality of life and healthcare use.
