First seen Jun 24, 2026 · Last updated Jun 27, 2026 · Updated 2 times

Summary

This study tests whether a coordinated care program called I-CONNECT can improve the health and well-being of older adults (65+) who have depression or bipolar disorder and at least two chronic physical conditions like diabetes or high blood pressure. Participants receive personalized care coordination, medication review, and regular follow-up from a team including their GP and psychiatrist. Researchers will compare their outcomes to those receiving usual care, focusing on quality of life and healthcare use.

What this could mean

Our plain-language read of the trial. This is informational only — not medical advice or a prediction.

Active substance

integrated care intervention (care coordination, personalized care plan, medication review, shared decision-making, regular follow-up)

What this could lead to

If it works, this could show that a coordinated care model improves quality of life and reduces hospital visits for older adults with depression and multiple health problems.

What could go wrong

This is a small early-stage trial with only 82 participants, so results may not apply to everyone. The intervention is complex and may be hard to deliver consistently.
