Summary

This study aimed to understand why some patients with heart failure end up back in the emergency room or hospital within 30 days of being discharged. Researchers looked at the medical records of 58 patients to find the main reasons for these quick returns. The goal was to use this information to suggest better follow-up care plans that could keep patients healthier at home and reduce costly, stressful readmissions.
