Summary

This study looked at two different types of follow-up care for people with heart failure after they leave the hospital. One group got a phone call from a care coordinator, while the other could request a home visit from a community paramedic who could connect them to a doctor by video. The goal was to see which approach better reduced hospital readmissions and improved patients' quality of life.
