Can extra help from pharmacists and health workers improve diabetes control?

Summary
This study looks at whether adding support from a pharmacist or a community health worker helps adults with type 2 diabetes better manage their blood sugar. About 376 people will be placed into one of four groups: usual care, pharmacist help, community health worker help, or both. The goal is to see which approach works best for improving diabetes control and medication use.
