Summary

This study looks at whether adding extra support from a pharmacist or a community health worker helps adults with type 2 diabetes better manage their blood sugar and medications. About 376 adults with uncontrolled diabetes will be placed into one of four groups: usual care, pharmacist support, community health worker support, or both. The goal is to find out which type of help works best for improving diabetes control.
