Summary

This study looks at whether giving extra support to community health centers helps them screen patients for non-medical needs like food or housing and refer them to community resources. Twenty health centers will be randomly assigned to get this support or not. The goal is to see if the support leads to more screenings and referrals for patients with diabetes or high blood pressure.
