Summary
This study tests a program that helps heart failure patients and their family caregivers work as a team to manage the disease at home. The program includes face-to-face and online support to improve self-care, quality of life, and reduce hospital readmissions. About 232 patient-caregiver pairs will take part after the patient is discharged from the hospital.
