Summary
This study tests a program called De-HF that helps heart failure patients and their family caregivers work together to manage the condition. The program includes both in-person and online support to improve quality of life and reduce hospital stays. About 232 patient-caregiver pairs will take part after the patient is discharged from the hospital.
