Summary
This study is creating and testing a patient navigation program to help young adults who have survived cancer. The program aims to guide survivors to get the recommended primary care and cancer check-ups they need. Researchers will first adapt an existing program for young survivors and then test it with a small group to see if it's helpful and practical.
