Summary
This study tested whether a transition coach could help 16-17 year olds with Juvenile Idiopathic Arthritis or Inflammatory Bowel Disease prepare for adult medical care. 27 teens took part in a coaching program to build self-management skills. The goal was to see if the program was practical and acceptable, not to cure or treat the disease.
