Summary

This study tested whether a transition coach could help 16-17 year olds with Juvenile Idiopathic Arthritis or Inflammatory Bowel Disease move from pediatric to adult healthcare. 27 teens took part in a pilot program to see if the coaching was practical and well-liked. The goal was not to treat the disease, but to learn if this kind of support is feasible and helpful for managing their own care.
