Summary
This study is testing a virtual patient navigation program for 260 underserved adults who speak English, Chinese, or Spanish and have breast cancer or heart disease. The program uses a phone-based portal to connect patients with navigators who can help them manage their care. The goal is to see if this approach is usable and satisfying, and whether it can expand access to navigation services for people who might otherwise lack support.
