Summary
This study tests a home monitoring system called VIEWER for people with advanced chronic kidney disease (stage 5) or end-stage renal disease. The system includes a tablet, blood pressure cuff, scale, oxygen monitor, and motion tracker. Researchers want to see if using this platform alongside usual care reduces emergency room visits and hospital stays.
