Summary
This study compares two telehealth approaches—digital health coaching and remote patient monitoring—to see which works best for managing type 2 diabetes in people with physical disabilities and at least one other chronic condition. About 5,000 participants will be followed for 12 months to measure changes in blood sugar levels and diabetes-related distress. The goal is to find the most effective and practical telehealth strategy for underserved groups to improve health equity.
