Summary
This study tests whether using a continuous glucose monitor (CGM) with the help of a Patient Navigator can improve diabetes management and quality of life for people with type 1 diabetes. The study includes 150 participants aged 14 to 64 who have had serious diabetes complications. The goal is to see if this support helps people use their CGM more consistently and feel better about their health.
