This study looks at whether giving parents of children with chronic conditions a mental health questionnaire during clinic visits is practical and helpful. About 100 parents will fill out the survey and receive a list of support services. Researchers will follow up at 3 and 6 months to see if parents found it acceptable and if they used any services.
