Summary
This study tests a smart management system that uses a WeChat app and home devices like blood pressure monitors to help people who have had a stroke or mini-stroke avoid another one. About 4,490 participants will either get usual care or use the system after leaving the hospital. Researchers will check if the system reduces deaths, strokes, and other heart problems over one year.
