Summary
This study tests if portable devices like handheld ultrasound and blood sugar monitors can find chronic diseases such as diabetes, heart disease, and kidney disease earlier than usual. 200 adults aged 18-75 will use these devices or standard care for 6 months. The goal is to see if these tools improve detection and health outcomes in primary care settings.
