Summary
This study tested a new way to care for people with heart failure after they leave the hospital. Over 500 patients used a home monitoring system to track symptoms and weight, along with nurse education and a follow-up visit. The goal was to see if this approach could reduce hospital readmissions and improve survival in older, frail patients.
