Summary
This study tests whether nurse-led education for heart failure patients and their informal caregivers can improve patients' quality of life, self-care, and medication adherence. About 280 patients recently hospitalized for heart failure and their caregivers will receive oral sessions, educational materials, and regular phone calls. The goal is to see if training both patients and caregivers together leads to better outcomes than training patients alone.
