Summary
This study tests whether a 3-month support program, led by a nurse and a community health worker and guided by an AI tool called MEDBRIDGE, can help people with type 2 diabetes manage their medications after leaving the hospital. The program aims to reduce confusion about medication changes and prevent emergency visits. Forty-five adults from two Alabama hospitals will take part to see if the program is practical and acceptable.
