App and nurse team aim to keep dementia patients at home longer

Summary
This study tests a program where specially trained nurses and a smartphone app support family caregivers of people with dementia. The goal is to help keep the person with dementia living at home longer, reduce caregiver stress, and manage difficult symptoms. About 554 families will take part, comparing this support to usual care.
