Summary
This study tests a program where specially trained nurses work with families caring for a person with dementia. The nurses help identify and address unmet needs, and a mobile app gives caregivers direct access to experts and memory clinics. The goal is to see if this support helps keep the person with dementia at home longer and reduces the caregiver's stress and burden. The trial will involve 554 families across multiple centers in Germany.
