Summary
This study tested a program called ENHANCE to help people with two or more long-term health problems like HIV, high blood pressure, diabetes, asthma, or depression. Over 1,800 adults aged 40 and older took part. Some clinics used a special decision-support tool and home visits from health workers, while others continued usual care. The goal was to see if the program could improve detection, treatment, and control of these conditions.
