Summary
This study tests a personalized home activity program for people with mild-to-moderate dementia who have just left the hospital. The program is designed to reduce behavioral and psychological symptoms, like agitation or mood changes. Participants will follow a tailored activity plan with help from a family caregiver for 12 weeks. The goal is to see if this approach improves symptoms and eases caregiver stress.
