Summary

This study follows 2,000 adults aged 45-75 in Pennsylvania to better understand why some groups have higher heart disease risk. About 800 participants at higher risk will receive either usual care or a personalized program with nutrition, exercise, and stress management advice. The goal is to see if such lifestyle support can reduce heart attacks and strokes.
