Summary
This study tests whether a virtual ward—using a tablet, scale, and blood pressure monitor at home—can reduce hospital readmissions for heart failure patients. 346 adults who were recently hospitalized for heart failure will either receive the virtual ward or standard care. Researchers will track hospitalizations, deaths, costs, and quality of life over 3 to 12 months.
