Virtual ward aims to keep heart patients out of hospital

Summary

This study tests whether a virtual ward — follow-up care via phone or video — can reduce hospital readmissions for people with heart failure. About 346 adults who were recently hospitalized for heart failure will take part. Researchers will compare readmission rates, costs, and quality of life between virtual ward care and standard care over 3 to 12 months.
