Summary

This study looks at whether a structured education program helps heart failure patients and their families learn about the condition, self-care, and emergency signs. About 1,000 adults hospitalized or treated for heart failure will take part. The program includes a one-hour session led by doctors and nurses, plus written materials. Patients fill out questionnaires before, after, and at 3, 6, and 12 months to measure knowledge and self-care habits.
