Summary
This study looks at whether 12 virtual health coaching sessions over 16 weeks can help people with heart disease (like heart failure or a heart attack) improve their health, lower stress, and reduce hospital readmissions. About 70 adults will take part, comparing coaching plus standard care to standard care alone. The goal is to see if coaching eases symptoms and helps patients manage their condition better.
