Summary

This study looks at how to make it easier for doctors to screen patients for physical inactivity and refer them to community exercise programs. Researchers will work with 35 clinics and their patients to see if this approach helps people become more active and improve health conditions like obesity, high blood pressure, and diabetes. The goal is to find the best way to make exercise a standard part of patient care.
