Summary
This study tested a new digital tool that scans patients' electronic health records to find older adults who might have undetected memory problems or be at risk for Alzheimer's disease. Researchers wanted to see if the tool was practical to use in primary care clinics and if sending messages through a patient app could successfully connect these at-risk individuals to a specialized nurse for further assessment. The goal was to help people get diagnosed earlier and connect them to appropriate care or research opportunities.
