New study aims to close racial gap in diabetes control

Summary
This study tests whether a special team-based program can help African American and Latinx adults with uncontrolled type 2 diabetes improve their blood sugar levels more than usual care. About 150 participants will either receive the team program or standard care with educational materials. The goal is to see if at least 30% of those in the program can lower their A1C by 0.5% in 6 months and keep it improved at 12 months.
