Summary

This study tests whether adding a trained community health worker (CHW) to standard care can improve quality of life and advance care planning for African American patients with advanced cancer. 160 patients will either receive standard care or standard care plus CHW support. The CHW helps with communication, understanding palliative care, and addressing social needs.
