Summary
This study tests whether a church-based program led by community health workers can improve heart health in Black communities more effectively than group education alone. About 812 adults aged 40 and older with heart disease risk factors will take part. The goal is to reduce racial inequalities in cardiovascular health by helping people manage blood pressure, cholesterol, and other risks.
