Community health workers may boost home dialysis use in High-Risk patients

Summary

This study looks at whether adding a community health worker to a kidney care team helps more patients with advanced kidney disease choose or start home dialysis. About 106 adults with severe chronic kidney disease will be split into two groups: one gets standard team care, the other gets the same plus a community health worker. Researchers will track home dialysis rates, patient engagement, and social needs over one year.
