Summary

This study tests whether a computer tool that uses continuous glucose monitor (CGM) data can help primary care doctors make better decisions for people with type 2 diabetes who use insulin. About 150 adults with type 2 diabetes and high blood sugar will take part. The goal is to see if the tool helps lower blood sugar levels and keeps them in a safer range.
