Summary
This study tests a new program designed to improve teamwork between cancer doctors and primary care doctors. It focuses on helping cancer survivors who also have high blood pressure, diabetes, or high cholesterol manage these conditions better during and after cancer treatment. The goal is to see if better communication and coordination between different doctors leads to improved health outcomes for patients.
