Summary
This study is testing a smartphone app designed to help older adults with multiple chronic conditions like heart failure, diabetes, and kidney disease manage their care after leaving the hospital. The goal is to see if the app can reduce complications and readmissions by providing clear plans for medications, appointments, and warning signs. Researchers will enroll about 800 patients to compare outcomes between those who use the app and those who receive standard care.
