Summary
This study is testing whether a special wearable device, guided by artificial intelligence, can help adults with chronic heart failure avoid emergency visits and hospital stays. About 205 participants will either wear the device for six months or receive standard care. The goal is to see if early alerts from the device lead to better health and fewer hospital trips.
