Summary
This study tests a simple 5-minute cognitive assessment called 5-Cog to help primary care doctors detect dementia and mild cognitive impairment earlier. It involves 6,600 older adults in 22 clinics in the Bronx and Indiana, focusing on communities that often face health disparities. The goal is to see if this quick test leads to more new diagnoses and better follow-up care within 90 days.
