This study tests a quick, 5-minute cognitive assessment called the 5-Cog to help primary care doctors identify older adults with undiagnosed dementia or mild cognitive impairment. The trial will involve 6,600 patients aged 65 and older across 22 clinics in the Bronx and Indiana.
