Summary
This study tests whether virtual group visits can help people with type 2 diabetes and related heart conditions better manage their blood sugar. About 720 adults with high A1C levels and at least one other cardiovascular risk factor will take part. The main goal is to see if these online sessions lower A1C, blood pressure, and cholesterol.
