Summary
This study tests whether virtual group doctor visits help people with type 2 diabetes and related heart problems better manage their health. About 720 adults with high blood sugar and at least one heart condition will take part. Researchers will check changes in blood sugar, blood pressure, weight, and cholesterol over time.
