Summary

This study is testing a mobile health system designed to help people with long-term health conditions like spinal cord injury, cerebral palsy, spina bifida, or traumatic brain injury improve their self-management skills and overall wellness. About 160 participants aged 12 and older who live in the community will use the app or receive standard care. Researchers will track changes in self-management, quality of life, and medical outcomes over 12 months.
