Summary

This study is creating a live registry using the health records of 40,000 heart failure patients within the Yale New Haven Health System. Its purpose is to monitor patients in real-time to identify gaps in recommended treatments and understand patterns in hospital readmissions. The goal is to use this information to improve the quality of care for people living with heart failure.
