Summary
This study tests a web-based support program called Hospital GamePlan4Care for caregivers of veterans with dementia who are discharged from skilled nursing facilities to home. Fifty-five caregivers will receive either the program (website, emails, and phone coaching) or basic health education. The goal is to see if the program is feasible and helps reduce caregivers' unmet needs and stress.
