Summary
This study tests a structured program called TransitionED designed to help youth aged 16+ with eating disorders move from pediatric to adult healthcare. The program includes planning packages, guided meetings, and a personal profile to share with new providers. Researchers will check if the program is practical and acceptable in real hospital settings, involving 60 youth, their caregivers, and clinicians.
