Summary
This study aimed to understand the challenges Spanish-speaking patients face when leaving the hospital for a nursing facility. Researchers interviewed 17 patients and caregivers to identify communication barriers and what helps or hinders a smooth transition. The goal is to use these insights to create fairer care processes for people with limited English.
